Formulations of 1% silver sulfadiazine in the treatment of burns

This work seeks to compile information on the use of pharmaceutical preparations for topical use containing the active ingredient Silver Sulfadiazine 1%. Its use is extended to different types of burns. In superficial burns (both first and second degree) the main objective is healing with the least scarring. In these wounds it is essential to achieve re-epithelialization. It is also important to keep the wounds moist to avoid scar formation. In deep burns, re-epithelialization does not occur and less scarring will occur if grafts are performed. Topical agents are currently applied during the short period between excision and then grafting. In pediatric patients, any hot fluid burn can generate significant injury because their skin is thinner. Although membranous dressings have a better response, in certain areas of the body it is easier to use topical agents. Regarding adverse effects, although there is not much information, silver may slow the healing process and may have cytotoxic activity on host cells. Renal toxicity has been reported with high concentrations of silver in blood and urine. Regarding associations with chlorhexidine, given the possibility that its presence may generate irritation in the area to be treated, we consider that priority should be given to the use of 1% Silver Sulfadiazine in cream only, without associating it with other active ingredients. If necessary, the wounds should be previously washed with chlorhexidine-based soap solution. In conclusion, this product can be used in different types of burns; it will be the job of the treating professional to choose when and how to administer it. It is important to consider that there have been described experiences of adverse reactions that should not be ignored at the time of indication.
